Reporting to the Senior Employment Facilitator the Program Coordinator is responsible for coordinating Skills Training services which provide people who have different abilities opportunities to increase skills and engage in their community--achieving the highest level of independence possible.
Shift: Full-time
Schedule: Monday-Friday
Hours: 8am-4:00pm
Pay: $19.25 per hour
Location: Alton, Illinois.
KEY RESPONSIBILITIES
Client Relations:
Promote a culture of person-centered services that advances the Company’s mission to provide quality care and services in a community-based setting to individuals with disabilities. Ensure a high rate of client satisfaction. Communicate effectively with clients regarding the progress toward goals and program success. Implement client-specific service requirements such as specific seating assignments, diet restrictions, mobility assistance, adaptive equipment, assistive technology, etc. Demonstrate our Company values to achieve optimum performance levels and achieve goals and objectives. Assist in the onboarding process for new clients. Lead the client safety committee and the client advocacy committee at assigned location.
Service Coordination:
Coordinate and communicate daily schedules including transportation, community supported employment, personal development classes, community integration, lunches and other skills training services and activities. Document movement of all Skills Training clients and staff during service hours.
Review client files and participate in training regarding clients individual needs. Execute programming for individuals including behavioral programming, document trails and provide update/feedback to Employment Facilitator regarding progress/needs. Develop training materials for assigned clients and employees.
Administrative Support:
Facilitate communication between managers and employees regarding essential information such as transportation route updates, changes in community customer needs, facility concerns, etc. Assist with transportation route time changes with client/family, preventative maintenance appointments, submission of receipts and documentation. Assist employees with technology needs and training. Provide support to Employment Facilitators with client goal documentation, filing, surveys, etc.. Act as a back up for Administrative Specialist for Skills training attendance/billing.
Documentation:
Complete incident/accident reports as needed in electronic record system. Address client issues and submit by end of shift. Submit all training log, documentation needed for training sessions or community integration. Use computer applications as required by agency procedures. Responsible for email and software usernames, and passwords by maintaining confidentiality. Prepare vehicles by completed all procedures such as pre-inspection checklist, fueling, mileage, and grant reporting sheets. Assist with grant related documentation and applications.
Client Employment, Training, Integration:
Train and assist clients to engage in work, training activities and community integration while monitoring progress, behavior and providing feedback/ re-direction as needed. Work with clients in learning and developing skills and abilities during work and non-working hours. Provide appropriate training during non-working hours by following the schedule and curriculum/materials.
Train/instruct clients in social, life and community skills. Accompany, train and support clients in community integrated activities like career exploration, community skills training, volunteerism, and community services. Help clients with assistance in the restroom, lunch and readying for bus runs. Complete janitorial and other facility needs as assigned.
Position Qualifications:
Education: High School diploma or G.E.D
Experience: 1+ years of Administrative Support (or other relevant) experience required. Experience working with people with disabilities helpful but not required.
Certifications/Licenses: CPR, 1st Aid, Crisis Prevention Institute (CPI), Direct Support Person (DSP) training provided by the company must be successfully completed within the first 4 months to be certified and annually thereafter for CPR, 1st Aid, and CPI to maintain the position.
Pre-Employment Tests: Basic Excel, Word, and Outlook
Computer Skills: Proficient with Microsoft Word, Excel, and Outlook.
Background Checks: Must pass criminal background check. Must pass various State and Federal registry checks. Must pass DCFS Abuse and Neglect Tracking System check Must pass driving history check and Company policy criteria, maintain valid driver’s license and be 25 or older to drive 15-person capacity van on Company business.
Driving: Use of personal insured vehicle for Company related travel is required. May occasionally drive Company vehicle to transport clients for community work or activities or for meetings.
Occasionally kneeling is required to perform CPR practice sessions, complete skills testing and use as required. Occasionally bending, reaching, and twisting; occasionally climbing stairs and/or ladders squatting.
